Muscle strength and longevity

Measures of muscular strength, especially handgrip strength, predict mortality risk in older adults. Separately, reported muscle-strengthening activity is associated with lower mortality, with the association levelling off at modest weekly amounts.

What the evidence shows

Systematic reviews of cohort studies find that lower handgrip strength is associated with higher all-cause mortality, independent of muscle mass.

A meta-analysis of muscle-strengthening activity found lower all-cause mortality among people reporting such activity, with the association appearing to plateau after roughly 30–60 minutes per week.

Strength is closely tied to physical function and independence in later life, which is part of the healthspan question rather than the lifespan question alone.

Strength as a marker, strength as a behaviour

This page carries two separate research questions that are easy to blur together. One asks what a measured capacity predicts; the other asks what a reported behaviour is associated with. They draw on different measurements and support different conclusions.

Measured grip strength

A few seconds with a dynamometer. Lower strength is associated with higher mortality in older adults, independent of muscle mass.

Reported strengthening activity

A behaviour recorded in questionnaires. Reporting such activity is associated with lower mortality, levelling off after roughly 30–60 minutes a week.

Grip strength is partly a general marker of health, so it may signal risk rather than cause it, and no trial has shown that raising grip strength or training with resistance extends human lifespan. What the literature supports more directly is the link between strength and physical function and independence in later life — a healthspan question as much as a lifespan one.
